The incidence of sepsis has been increasing because of the advancing age of the general population, a greater number of invasive procedures, and more immunosuppressive therapies. The mortality of sepsis is still high, and is still the major cause of morbidity and mortality for patients admitted to an intensive care unit. In sepsis, exaggerated responses might induce organ dysfunction including lung injury and shock.
Extravascular lung water (EVLW) estimates fluid in the pulmonary interstitial and alveolar spaces. Extravascular lung water (EVLW) can be measured at the bedside using the transpulmonary thermodilution technique and has a diagnostic value for the identification of patients. It is an indicator of prognosis and severity of sepsis-induced lung injury. EVLW measurement is useful in characterising the severity of respiratory disease and improves septic patient management. Increased EVLW is associated with worse survival in patients with sepsis. A reduction of EVLW at early treatment and a negative fluid balance is associated with a better outcome.
Keywords as sepsis, septic shock, lung injury and extravascular lung water are searched in various databases and the prognostic, diagnostic and clinical significance of EVLW in sepsis and sepsis induced lung injury are discussed in this review.
Sepsis sıklığı toplumun yaşlanması, girişimsel işlemlerin sayısının artması ve immünsüpresif tedavinin yaygınlaşması ile artmaktadır. Günümüzde yoğun bakım ünitelerindeki hastalarda sepsise bağlı morbidite ve mortalite sıklığı hala yüksek seyretmektedir. Sepsiste organizmanın vereceği abartılı yanıta bağlı olarak akciğer hasarı ve şok gelişebildiği gibi odağın akciğer olup olmadığı ile ilgili olmaksızın sıklıkla akciğer hasarı da gelişebilir.
Damar dışındaki akciğer suyu yaklaşık olarak akciğer intertistiyel ve alveoler alandaki sıvının toplamıdır. Hasta başında termodilüsyon yöntemi ile ölçülebilen ekstravasküler akciğer suyu sepsis kaynaklı akciğer hasarının ciddiyetini ve prognozu gösterebilen bir parametredir. Bu değerin ölçümü solunum sıkıntısının ciddiyetini belirlemekle birlikte sepsis hastalarının yönetiminde de faydalıdır. Sepsiste artmış bu sıvıya bağlı yaşam beklentisi azalmaktadır. Ekstravasküler akciğer suyunun erken dönemde azaltılması ve negatif sıvı dengesi ile daha olumlu sonuçlar elde edilmektedir.
Bu derlemede, çeşitli veri tabanlarına sepsis, septik şok, akciğer hasarı ve EVLW anahtar kelimeler olarak verilerek ekstravasküler akciğer suyunun sepsis ve sepsis kaynaklı akciğer hasarında klinik önemi, diagnostik ve prognostik değeri irdelendi.
