OBJECTIVE: In this study the aim was to determine the etiological agents of nasocomial infections and pathogens in intensive care unit.
METHODS: We studied on 189 patients who were treated more than 72 hours in our intensive care unit (ICU). Sputum, tracheal aspirate, urine sample, swab from any wound and blood sample with a concomittent fever higher than 38ºC were collected to evaluate the infectious status during the patient’s first admission into the intensive care unit. The previous prescribed antibiotic regimen has been continued till the culture results obtained.The patients were followed up prospectively daily together with infectious disease specialists. Nasocomial infections were defined based on CDC criteria (Centers for Disease Control and Prevention).
RESULTS: Totally, there were 42 patients who developed 77 episodes of infection. The types of infections were as follows: ventilator-associated pneumonia (VAP) (n=37, 45.5%), urinary tract infections (n=23, 29.9%), pneumonia (n=18, 23.4%), primary bacteremia (n=2, 2.6%). The most frequent pathogens were Acinetobacter spp. (27.3%), Pseudomonas aeruginosa (19.5%), Stafilococcus aureus (18.2%), E.coli (16.9), Candida spp. (16.9%), Enterococcus spp. (11.7%). Resistance to methicillin was 71.4% among S.aureus isolates, without any resistance to vancomycin. The most frequent pathogens in pneumonia patients; P. aeruginosa (27.8%) and methicillin resistance S.aureus (MRSA) (22.2%) was determined. The most frequent pathogens in VAP patients; Acinetobacter spp. (48.6%) and P.aeruginosa (20.0%), in urinary tract infections; Esherichia coli (40.9%) and Enterococcus spp. (40.9%) was found. MRSA was determined in both primary bacteriemia patients.
CONCLUSION: Monitoring of nosocomial infections, infectious agents and resistance rates should be evaluated together. Surveillance studies should be performed regularly, besides appliying effective infection control policies so that the prevention of infections in ICU can be effectively controlled.
AMAÇ: Bu çalışmada yoğun bakım ünitesinde (YBÜ) gelişen hastane infeksiyonlarının tipleri, etken patojenleri ve bu patojenlerin antibiyotik direnç profillerinin belirlenmesi amaçlandı.
YÖNTEMLER: Hastanemiz YBÜ’de 72 saatten fazla tedavi gören 189 hasta değerlendirilmeye alındı. Tüm hastalardan YBÜ’ye alındığında balgam veya trakeal aspirasyon materyali, idrar, ateş 38,0 C ve üzerinde ise kan örneği ile mevcut olan herhangi bir akıntı veya yaradan örnek alınarak kültür-antibiyograma gönderildi. Antibiyogram sonucuna kadar hastanın varsa mevcut antibiyoterapisine devam edildi. Hastalar her gün düzenli olarak İnfeksiyon hastalıkları uzmanı ile birlikte değerlendirildi. Hastane infeksiyonunun belirlenmesinde CDC (Centers for Disease Control and Prevention) ölçütleri kullanıldı.
BULGULAR: Çalışmamızda 42 hastada 77 infeksiyon atağı saptandı. Hastane infeksiyonlarının dağılımı sıklık sırasına göre: ventilatör ilişkili pnömoni (VİP) (n=35, %45.5), üriner sistem infeksiyonu (ÜSİ) (n=23, %29.9), pnömoni (n=18, %23.4) ve primer bakteriyemi (n=2, %2.6) şeklinde saptandı. Etken bakteriler; Acinetobacter suşları %27.3, Pseudomonas aeruginosa %19.5, Stafilococcus aureus %18.2, E.coli %16.9, Enterococcus türleri %11.7 ve Candida %16.9 olarak izole edildi. S.aureus suşlarında metisilin rezistansı %71.4’di, vankomisin direnci saptanmadı. Pnömoni vakalarında en sık; P. aeruginosa (%27.8) ve metisiline dirençli S.aureus (MRSA) (%22.2), VİP vakalarında en sık; Acinetobacter spp. (%48.6) ve P. aeruginosa (%20.0), ÜSE’de ise en sık; Esherichia coli (%40.9) ve Enterococcus spp. (%40.9) saptandı. İki primer bakteriyemi olgusunda da MRSA etken patojendi.
SONUÇ: YBÜ’de infeksiyonların kontrol altına alınabilmesi için her hastanede infeksiyon etkenleri ve direnç profilleri düzenli olarak izlenmeli ve bu sonuçlar ışığında ileriye dönük etkin enfeksiyon kontrol hedef ve planları yapılmalıdır.
