Açık Kalp Cerrahisinde Postoperatif Solunum Sistemi Komplikasyonlarının Preoperatif, İntraoperatif ve Postoperatif Belirleyicileri

AMAÇ: Riskli kalp operasyonların giderek yaygınlaşması ve değişen hasta profili intraoperatif ve postoperaif dönemde daha fazla sorunla karşılaşılmasına neden olmaktadır. Postoperatif dönemde görülen solunum sistemine ilişkin sorunlar mortalite ve morbiditenin artmasında en önemli nedenler arasındadır. Çalışmamızın amacı açık kalp cerrahisi olgularında gelişen solunum sistemi komplikasyonlarını belirlemek ve bunlara neden olan preoperatif, intraoperatif ve postoperatif belirleyicileri saptamaktır.
YÖNTEMLER: 2000-2005 yılları arasında gerçekleştirilen 1401 açık kalp cerrahi geçiren olgunun dosyaları retrospektif değerlendirildi. Hastaların cinsiyeti, yaşı, vücut ağırlığı, boyu, preoperatif laboratuvar değerleri, sol ventrikül ejeksiyon fraksiyonu(LVEF, %), yandaş hastalıkları, β-bloker kullanımı, sigara kullanımı, operasyon tipi ve süresi, kardiyopulmoner bypass(KPB) süresi, anestezi süresi, intraoperatif transfüze edilen kan vb ürünler, postoperatif ekstübasyon zamanı, mediastinal drenaj miktarları, yoğun bakım ve hastanede kalma süreleri kaydedildi. Bu belirleyiciler ile postoperatif solunum sistemine ilişkin komplikasyonlar arasındaki ilişki araştırıldı.
BULGULAR: Çalışmamızda, açık kalp cerrahisi olgularında postoperatif solunum sistemine ait komplikasyonların sıklığı %39,2 olarak saptandı. En sık görülen komplikasyon uzamış mekanik ventilasyon (%36,1) iken, en az görülen komplikasyon ise pnömotoraks (%0,6) idi. Solunum sistemi komplikasyonları görülen olgularda mortalite oranı %3,3 olarak bulundu. Solunum sistemi komplikasyonları için risk faktörleri olarak; ileri yaş, kadın cinsiyeti, sigara kullanımı, hipertansiyon(HT), kronik obstrüktif akciğer hastalığı(KOAH), kombine operasyon, preoperatif hematolojik ve biyokimyasal değerlerin normal olmaması, KPB, anestezi ve operasyon süreleri, ekstübasyon, yoğun bakım ve hastanede kalma süreleri, intraoperatif kan vb ürünlerin kullanımı ve mediyastinal drenajın fazla olması olarak bulunmuştur.
SONUÇ: Preoperatif dönemde hastaların ayrıntılı incelenmesi ve optimal sağaltımın yapılması, KPB, operasyon ve anestezi sürelerinin kısaltılmasını, uygun fast-tract anestezi yönteminin seçilmesini sağlayarak solunum sistemi komplikasyonlarının ve mortalite oranlarının azaltmasını sağlayabilir.

Preoperative, İntraoperative and Postoperative Predictors of Postoperative Respiratory System Complications in Patients Undergoing Open Heart Surgery

OBJECTIVE: The increasing prevalence of risky cardiac operations and the changing patient profile,can cause to encounter more problems during intraoperative and postoperative periods.Problems related to the respiratory system in the postoperative period are most important reasons of increasing mortality and morbidity rates.The aim of study was to determine the preoperative, intraoperative, and postoperative predictors of the respiratory complications of open-heart surgery patients.
METHODS: 1401 patients undergoing open-heart surgery between2000-2005, are evaluated retrospectively.Patients’ age, sex, body mass index, preoperative laboratory tests, left venricular ejection fraction(LVEF, %), comorbid diseases, use of β-blocker,smoking,type and duration of operation, duration of cardiopulmonary bypass and anesthesia, transfusion of blood intraoperatively, time of extubation, mediational drainage, length of stay in the ICU and in the hospital were recorded,.The relationship between these predictors and postoperative respiratory complications is investigated.
RESULTS: In our study, the prevalence rate of postoperative respiratory complication among patients undergoing open-heart surgery was found to be39,2%.The most common complication was prolonged mechanical ventilation(36.1%), while the least frequent complication was pneumothorax(0.6%). The mortality rate due to respiratory complications was%3.3.As risk factors for respiratory complications included older age, female, smoking, hypertension, COPD, combined operations, preoperative hematologic and biochemical abnormalities, duration of CPB, anesthesia and operation time, extubation, ICU and length of hospital stay, intraoperative transfusion of blood products and mediastinal drainage.

CONCLUSION: The detailed examination of the patients preoperatively and optimal treatment, could provide reducing complications of anesthesia and mortality rates, by allowing selection of the appropriate method of fast-tract respiratory system and shorter duration of surgery, anesthesia and CPB.
