The effect of patient position and pneumoperitoneum on perfusion index and pleth variability index in patients undergoing laparoscopic cholecystectomy

INTRODUCTION: Patient position and pneumoperitoneum during laparoscopic surgery effect hemodynamic parameters by changing intrathoracic pressure. The aim of the study is to investigate the effect of reverse Trendelenburg and pneumoperitoneum on perfusion index (PI) and pleth variability index (PVI) during laparoscopic surgery.
METHODS: Seventy-five patients who would undergo elective laparoscopic cholecystectomy under general
anaesthesia for cholelithiasis in two centres were recruited in this prospective observational study. Respiratory
and hemodynamic parameters, PI and PVI values were recorded prior and after the anaesthesia induction,
during reverse Trendelenburg position, during pneumoperitoneum and after deflation
RESULTS: Due to haemodynamic instability that required medication 7 patients were excluded from the study. PI values increased after general anesthesia induction (P<0.001) but PVI values didn’t change (P>0,05). No significant change is observed in PI values with the patient position and pneumoperitoneum. PVI values measured in reverse Trendelenburg position and pneumoperitoneum were higher than post-entubation values (P<0,05) and post-deflation values were similar to baseline values (P>0,05).
DISCUSSION AND CONCLUSION: PI values increase, and PVI is not affected during general anaesthesia. Reverse Trendelenburg position and pneumoperitoneum do not cause a change in PI values, whereas PVI values increase These changes should be taken into consideration when fluid management is done by PI and PVI monitoring during surgeries done with pneumoperitoneum or in reverse Trendelenburg position.

Laparoskopik kolesistektomide hasta pozisyonu ve pnömoperitonyumun perfüzyon indeksi ve pleth değişkenlik indeksi üzerine etkileri

GİRİŞ ve AMAÇ: Laparoskopik cerrahide kullanılan hasta pozisyonu ve pnömoperitonyum intratorasik basıncı değiştirerek hemodinamik monitörizasyon parametrelerini etkileyebilmektedir. Bu çalışmanın amacı, laparoskopik kolesistektomi ameliyatlarında uygulanan ters trendelenburg ve pnömoperitonyumun perfüzyon indeksi (PI) ve pleth değişkenlik indeksini (PVI) nasıl etkilediğini araştırmaktır.
YÖNTEM ve GEREÇLER: Prospektif gözlemsel çalışmaya kolelitiyazis nedeniyle genel anestezi altında elektif laparoskopik kolesistektomi ameliyatı geçirecek iki merkezde toplam 75 hasta alındı. Anestezi indüksiyonu öncesi ve sonrasında, ters trendelenburg sırasında, pnömopertioneum uygulandığında ve desuflasyon sonrasında solunumsal ve hemodinamik parametreler ile PI ve PVI değerleri kaydedildi.
BULGULAR: Tedavi gerektiren hemodinamik instabilite nedeniyle 7 hasta çalışma dışı bırakıldı. Genel anestezi indüksiyonu sonrası PI değerleri arttı (P<0,001) ancak PVI değişmedi (P>0,05). PI değerlerinde hasta pozisyonu ve pnömoperitonyuma bağlı anlamlı bir değişim gözlenmedi. Ancak ters trendelenburg pozisyonu ve pnömoperitonyum yapıldığında ölçülen PVI değerleri entübasyon sonrası ölçülenden yüksekti (P<0,05) ve desuflasyon sonrası başlangıç değerlere benzer bulundu (P>0,05).
TARTIŞMA ve SONUÇ: Genel anestezi altında PI artmakta ancak PVI değişmemektedir. Hem ters trendelenburg pozisyonu hem de pnömoperitonyum uygulanması PI’da değişikliğe yol açmazken PVI’yı arttırmaktadır. Pnömoperitonyum uygulanan veya ters trendelenburg pozisyonu verilen ameliyatlarda PI ve PVI monitörizasyonu ile sıvı yönetimi yapılırken bu etkiler göz önünde bulundurulmalıdır.
